If the mechanism is binding up and not allowing the lever to move it could need to be lubricated. It can take a pretty significant cleaning process to completely clean out the weld nuts for installation on a hitch like the Draw-Tite # 76028. You can use a wire brush like # 814092 like you did but if it wasn't enough you'll need to take an extra step.

The next step is to inspect the trailer connector for corrosion in the connector, inspect the wiring and where the harness connects to the truck wiring for any shorts.

The first step to take is to loosen the draw-down bolt on your Companion # BWRVK3500 or # BWRVK3400. Without this being loose the pull pin won't disengage.

I can say that the Draw-Tite # 75715 is a fit for your 2012 Nissan NV 2500 and I included an install we did on this exact vehicle. With that said, what I think is going on is the hitch is flexed a little. This does happen pretty often as the metal side brackets are not a super rigid steel. As to the flange not sitting flush like it should, this is actually a fairly common occurrence with hitches. The frame brackets on hitches are a mild steel so they can flex with the frame of your vehicle when it twists over bumpy terrain. Sometimes hitches can get slightly bent during shipping and this can be fixed by bending the hitch back into place as long as the hitch only has a slight bend to it. If yours isn't lining up I can almost guarantee that is what happened. Some people will use open steps for leverage, vices and even car jacks to apply controlled force to bend the hitch back.
